This worksheet is provided for individuals who do not have access to a computer, or who prefer to think and work using real paper instead of a computer.

To complete the survey, answer each of the questions below. For each question, check the box from 1 to 5 that best describes you, where the number means:

4: Frequently true of me
5: Very true of me, consistently

When you are completed, either use this worksheet to fill in the form on line, or bring it to church and someone will enter it for you. After your answers have been entered, you will be given a printed copy of your spiritual gifts assessment.
